Job Summary
We are seeking a skilled Licensed Practical Nurse to join our team at UW Health Rehabilitation Hospital. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our interdisciplinary team. The ideal candidate will have a passion for rehabilitation nursing and a commitment to delivering exceptional patient outcomes.
Key Responsibilities:

• Provide direct patient care, including administering medications, assessing patient needs, and developing individualized care plans.
  
• Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
  
• Participate in quality improvement initiatives to enhance patient care and safety.
  
• Demonstrate a strong commitment to patient-centered care and a positive, supportive attitude towards patients and colleagues.
  
  Requirements:
  • Graduate of an accredited Licensed Practical/Vocational Nurse program.
    
  • Current state licensure as a Licensed Practical/Vocational Nurse.
    
  • CPR/BCLS certification, ACLS preferred.
    
  • IV Certified or obtained within 6 months highly preferred.
    
  • Minimum six months Medical/Surgical experience in an acute care setting preferred.
