Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate complex care management activities for members.
Key Responsibilities
Evaluate member needs and recommend plans for optimal healthcare
outcomes
Develop ongoing care plans and collaborate with providers to
address unmet needs
Provide resource support and education to members and their
families on healthcare options
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
Must reside in the Eastern Standard Time (EST) zone
